I, the undersigned participant, parent or guardian of a minor, do hereby consent to my or my child’s participation in Recreational Programs of the Town of Longmeadow.
I expressly understand and agree that the Recreational Programs may present risks, both serious and minor, including but not limited to head or other injuries, loss of sight, broken bones, brain damage, paralysis, illness, and death, including any death, illness, injury and/or disease in any way related to or arising out of the novel coronavirus (COVID-19). I am responsible for researching and evaluating the risks that I/my child may be faced with and am responsible for my/my child’s actions. I attest that I/my child is physically capable of participating in the Recreational Programs, have no known health restrictions that might jeopardize my/my child’s safety or health or the safety or health of others while participating in the Recreational Programs, and that I have medical insurance.
I agree to forever release the Town of Longmeadow, its employees, agents, board members, volunteers, assigns, and any and all individuals and organizations assisting or participating in any voluntary or recreational programs of the Town (hereinafter collectively “the Town”) from any and all claims or any nature whatsoever, rights of action and causes of action that may have arisen in the past, or may arise in the future, directly or indirectly, from any and all known or unknown personal injuries to myself or to my child before or after he or she reaches the age of maturity (if guardian) or property damage resulting from or growing out of my or my child's participation in the Town of Longmeadow voluntary Recreational Programs.
I hereby release, indemnify, defend, and hold harmless the Town against any and all legal claims or any nature and proceedings of any description that may have been asserted in the past, or may be asserted in the future, directly or indirectly, arising from personal injuries to myself or my child or property damage resulting from participation in the Town of Longmeadow voluntary Recreational Programs.
I further affirm that I have read this Consent and Release Form and that I understand the contents of this Form. I understand that participation is voluntary and that I am free to choose not to participate in said Recreational Programs. By signing this Form, I affirm that I have decided to participate in the Recreational Programs with full knowledge that the Town will not be liable to anyone for personal injuries and/or property damage that I or my child may suffer by participating or volunteering in the Town Recreational Programs.
THE LONGMEADOW PARKS AND RECREATION DEPARTMENT MAY VIDEOTAPE OR TAKE PHOTOGRAPHS OF PARTICIPANTS ENROLLED IN RECREATION ACTIVITIES, CLASSES OR PROGRAMS. THESE PHOTOS AND/OR VIDEOS MAY BE USED FOR PROMOTIONAL PURPOSES.
